Sotoyome Campus Guide

This "Patient and Family Guide" will answer many questions about your inpatient stay at the Sotoyome Campus of Santa Rosa Memorial Hospital. Please contact your case manager or nurse with any other concerns. A phone directory (at the end of this text) is available for your convenience.

Our Primary Services

Skilled Nursing Services

Interdisciplinary, rehabilitative care is directed by a team of community physicians treating neurological, pulmonary, and oncology issues, as well as cardiac, wound care, and a variety of other conditions.

Palliative Care Center

This small but cozy unit at the Sotoyome Campus of Santa Rosa Memorial Hospital is the first of its kind in Sonoma County. We provide specialized inpatient care for patients who are experiencing a life threatening illness and require acute pain and symptom management or respite care to improve their quality of life.

Admissions

Before being admitted to SRMH - Sotoyome Campus, your physician will write an order to admit. To facilitate the admissions process, please bring your Medicare and/or insurance cards. . Some insurance plans require pre-certification prior to admission. Please follow all requirements set forth in your insurance plan and make any necessary arrangements. If you are responsible for a co-payment or deductible, you may be asked to make this payment at the time of admission.

Case Management

Your case manager will assist you and your family. Case Management provides the following:

  • Coordinates services as authorized by your insurance carrier.
  • Obtains prior authorization from your insurance for any specialty services or specialty equipment
  • Provides information and appropriate documentation to your insurance company
  • Coordinates caregiver training for family members who will provide patient care following discharge.

What to Bring

Patients being admitted for medical services should not pack street clothing. If dressing is a focus of your treatment, we will discuss with you the type of clothing you need.

Plan to bring the following:

  • Undergarments, socks, pajamas, bathrobe, slippers (optional), shoes (sturdy, comfortable, with low heels), toiletries, sweater (cardigan style).
  • Other equipment such as wheelchair, splints and braces you customarily use; eyeglasses, hearing aid and extra batteries, dentures. Be sure to store these items in appropriate containers to prevent loss.

Please do not bring jewelry, watches, money, or other valuables to the hospital. The hospital is not responsible for lost items.

Spiritual Care

Our Spiritual Care Department is an integral part of the health care team offering a holistic ministry for the spiritual, emotional and ethical concerns of our patients and their families. Our chaplains include Catholic priests and sisters, ministers of Protestant denominations, and laypersons. Chaplains are available to support you during your time of illness or crisis, to counsel and pray with you and your family and to administer sacraments. Our chaplains would be happy to contact your personal minister or rabbi upon request.

Telephones

To make a local telephone call, dial ““ for an outside line, followed by the number you wish to reach. To place a long-distance call within the 707 area code, dial “9-0”, and number. To make a long-distance call outside the 707 area code, dial “9-0” followed by the area code and the telephone number. For long-distance calls, you may call collect, bill your home phone or use a phone card. Calls cannot be charged to your hospital bill.

Activities

Please refer to the activity schedule posted on the nursing units for a listing of planned activities.

Meal Services

Meals will be served to you each morning between 7:45 a.m. and 8:30 a.m. Lunch is served between 12:15 p.m. and 1 p.m. and dinner is served between 5:45 p.m. and 6:30 p.m. A menu will accompany your daily breakfast tray so that you can select your name and room number when completing your menu request. If you need help filling out your menu, a staff member will be happy to assist you.

If the selections offered do not meet your needs, please ask to speak to the dietitian. We can usually honor special requests if the items meet your therapeutic diet restrictions as ordered by your physician.

Restrooms

Patient bathrooms are for patient use only. Public restrooms are located near the cafeteria.

Newspapers

You may purchase "The Press Democrat" from the rack located outside the front entrance of the building.

Laundry

Due to our limited facilities, please plan to have a family member clean your personal laundry every three days. Soiled items will be placed in a plastic bag and stored in your closet. Hospital linen is laundered off site, so please do not mix personal clothing with bedding.

Family Room

We invite visitors of patients at the Sotoyome Campus to gather and relax in our Family Room, which features a lounge area, a kitchenette, reading materials and a children’s play area. The Family Room is located at the north end of the building (in the Palliative Care wing), and is open 24 hours a day.

Visitor Dining

Visitors may purchase meals from the cafeteria at the Sotoyome Campus during the following times:

Breakfast 8:30 – 9 a.m.
Lunch 12:30 – 1 p.m.
Dinner 6 p.m. – 6:30 p.m.

Snacks and beverages may be purchased anytime from the vending machines located in the cafeteria. Visitors may bring meals to your room or dine in the cafeteria. The Creedside Café at the main hospital campus on Montgomery Drive is also available for visitors from 7 a.m. until 7:30 p.m. (Weekend hours may vary.)
